Booking for more than one?
If you need to book for more than one person please make a separate copy of this form yourself or contact Karl Goswell for additional copies.

Cancellation Policy
If you have to cancel your place before January 31st a full refund of money will be paid to you, less the non-refundable deposit of £30.
If cancelling between February 1st—28th, 50% of your fees will be payable.
If cancelling between March 1st—24th, 75% of your fees will be payable.
If cancelling after March 25th, full fees will be payable.

Accommodation: Accommodation is in single rooms. If you have any mobility problems and would prefer accommodation closer to the main conference facilities, or if you require ground floor accommodation, please indicate here:
